Possible questions for a questionnaire

Maybe we could use this page as a central place to try to combine our efforts in coming up with possible useful questions for the case study and have a discussion about these questions. The final questions that you will use as a group will of course be aligned to your specific situation and organization, but a sufficient number of questions could be relevant for everyone I think. The aim of the questions should be to try to get insight in the phases a company is going through in the adaptive cycle.
The initial structure below is the one suggested in the lecture of 19-3. Please feel free to add any questions, substructures, comments or suggestions that you think could be relevant, better too much than too little questions.
Company confidentiality needs to be ascertained and adhered to.


  1. What industry does your organization operate in?
  2. Is your organization for profit or non-profit?
  3. How would you describe your core business?
  4. What kind of services and/or products does your organization offer?
  5. How would you describe your business model?
  6. Is your organization currently going through any change processes/reorganizations?
  7. Do you expect to initiate change processes in the near future?
  8. How do you think your company handles changes?
  9. How did you involve your employees and keep them informed?
  10. How did you reflect previous learning experiences? And how are you using them in the future?
  11. What characteristics of the system are vulnerable to a crisis event?
  12. Are you actively looking for new opportunities for creating business at the moment? (for example R&D activities)
  13. In what environment does your organization operate? Is it highly dynamic or more static?
  14. How is the competition? Are you confronted with aggressive competition?
  15. Is your organization faced with declining sales/revenue?
  16. What (combination of) organizational configurations (Mintzberg terminology) best fits the organization?
  17. What is the dominant culture in your organization?
  18. What is the dominant management behavior? (e.g. "normal" or more innovative)
  19. Is there a common information architecture available within the organization (Yes/no)
  20. How old is the average user of the organization-wide applications?
  21. How large is the installed user base? (number of users)
  22. Is your organisation highly 'information based' or less so?


  1. How is strategy defined in your organization? (strategic management process) e.g. top down or more bottom up/emergent.
  2. Have there been any recent changes in your strategy?
  3. How does your organization respond to changes in its environment? i.e. emerging trends or innovative practices in the external environment?
  4. Is there a common information architecture available within the organization? (Yes/no)
  5. Does the organization have a ‘make’ or ‘buy’ attitude?
  6. Does the organization do their own integration or is that outsourced?
  7. Does your organisation involve the majority of employees in business decision-making? (brain-storming)
  8. Does your organisation encourage employees to be innovative? In what way?
  9. Does the organization have representatives abroad or does it have it's own offices there?
  10. Does your organisation have an outside-in-view or inside-out-view approach?
  11. Is your organisation familiar with the Cynefin decision-making framework on complexity and management? Do you use it?
  12. Does your organisation use any form of Psychometric testing to align employee strengths to the organisations vision and mission? (Spiral Dynamics, etc)
  13. Would your organisation redeploy or retrench employees during/after significant changes to the organisation?
  14. Does your organisation have an active business plan? Would you be prepared to let us have it for research purposes?
  15. Does your organisation have a SWOT analysis of pre- and post- change? Would you be prepared to let us have it for research purposes?
  16. How resilient do you think your company is to change? (embracing change, adapting and coming through strong then moving back to equilibrium status)
  17. If your organisation went through disruptive change, would the organisation stay functional as a unit or create new (smaller) spin-off, independent organisations or units?


  1. Are there currently any highly standardized (factory like) processes implemented and being exploited in your organization?
  2. Do you register and keep track of changes on your operational business?
  3. Are there specialists involved within your daily operational business process?
  4. Are all operational processes described in work-instructions or procedures?


  1. Is there a common information architecture available within the organization? (Yes/no)
  2. Is the data and/or application compartmentalized within the organization?
  3. Are there legacy systems in operation?
  4. Are operations and applications standardized or specific (tailor-made)?
  5. Does the organisation make use of software packages, and if so which major ones? (Accounting packages..., JD Edwards, SAP, Oracle, etc)
  6. What is the average lifespan of (organization-wide)applications?
  7. Which formal communication channels does the organisation use? Please give a list (e.g. company address, meetings, paper notices, email, web site, blogs, Twitter, other)
  8. Does the organisation use social media for formal communication?
  9. What informal communication channels are you aware of that are used by employees to vent frustrations? (blogs, twitter, facebook, sms,'borrels', etc)
  10. Does your organisation use virtual presence for meetings or training sessions? (Telepresence, Webex, V-talk, Skype, etc)
